Johns Hopkins University CO Nurse Practitioner in Baltimore, Maryland
The Department of Medicine, Division of Geriatrics at Bayview Medical Center seeks a Nurse Practitioner. The Johns Hopkins Home-based Medicine (JHOME) Program has been providing medical care for home bound older adults for more than four decades. Our mission statement is “To work together to build a future at home for our older patients, their families, and community” . The addition of a nurse practitioner enables us to broaden the scope of our current services and expansion of our patient panel. The Nurse Practitioner will be an integral member of the JHOME team and also work within the Division of Geriatrics to improve services provided to the home bound population.
Specific Duties and Responsibilities:
Responsible for the coordination of the medical care of the patients receiving primary medical care at home. Screen patients for program eligibility, conduct in-home health assessments and participate in our weekly inter-disciplinary team meeting. The Nurse Practitioner works cooperatively with the physician trainees including medical students, physician residents, and geriatric medicine fellows.
- Clinical Duties:
Perform routine scheduled, urgent, and emergent medical evaluations and coordinate medical consultations. Perform annual wellness and dementia screening. Administer lab draws and vaccinations/immunization at home, if needed. Perform home safety, caregiver and functional assessments. Provide therapeutic counseling related to medical management. Prescribe and order controlled and no controlled substances. Counsel and refer to hospice and palliative care. Order necessary diagnostic for treatment and management of diseases. Removal of sutures, IV lines and staples. Wound assessment and management.
- Coordination of Medical Services:
Act as the coordinator for patients, patients' families, referring physicians, and third party carriers with regard to medical issues. Work with others in a team environment. Refer to home services and hospice. Respond to In basket messages and phone calls from patients. Track patient’s hospitalizations and communicates with inpatient attending.
- Productivity Targets:
Productivity targets associated with charges, payments, and visits have been developed for your clinical activities and it is the expectation of the Division that you will strive to practice at or above the target levels for your services.
- Inter-disciplinary Team Participation:
Participate in the inter-disciplinary team meetings and effectively represent the medical aspects that impact the patient care goals.
- Teaching Duties:
Serve as a clinical preceptor for residents, fellows and nurse practitioner students who are enrolled in degree programs at approved universities/colleges.
- Documentation of Medical Services:
Complete the written documentation of medical services required by regulatory agencies including the Maryland Office of Health Care Quality, Joint Commission for Accreditation of Health Care Facilities, the Centers for Medicare and Medicaid Services, and the third party insurers identified by the patients, within 48 hours of date of service.
- Divisional Cross-coverage
Provide cross-coverage to other Geriatric units such as Specialty hospital, when needed.
Master's degree in nursing; Registered Nurse license and Certified as a Nurse Practitioner. Must be licensed as NP in State of Maryland or other state where practicing. Three to five years NP inpatient or outpatient practice preferred. Current CPR certification required. Must maintain current licensure and certification during duration of employment. Completion of the Johns Hopkins Hospital and Bayview Medical Center credentialing process is required prior to start date.
Additional Knowledge, Skills and Abilities:
General medical experience with geriatric, medically complex and high acuity patients desired. Computer skills to use the Johns Hopkins electronic patient records systems.
Sitting, standing and walking for extended period. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s). Ability to move standard equipment through a hospital or clinical environment.

This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
